**Job Title: Post 16 Transition Lead (Level 3)**
**Hours: 20** hours per week (opportunity to undertake over time)
**Pay**:£14.50 p/h
**Holiday**:15 days per year plus bank holidays
**Contract**:Permanent
**Education & Qualifications**:Higher qualifications relevant to the job role.
**Job Purpose**:
- To work in-line with the organisation’s approach to relationship based, person-centred, individualised educational support.
- To work in conjunction with the Post 16 Development Lead, provide direct support to students, their families, their schools, and others involved with the person’s Post 16 educational support.
- To support the students within our Post 16 Provision to ensure the delivery of the day to day and future operations.
- To work in conjunction with the Post 16 Development Lead to develop the service.
- To support those young people who have historically struggled with formal education and the wider community.
- To support young people in their employment and future employment so that they become more independent and less reliant on the Welfare system. The purpose is to support these young people into adulthood and provide them with the means to succeed and contribute to society.
- To support the reduction of the number of young people who are deemed to be “Not in education, employment or training” (NEET)
**Responsibilities**:
- To liaise with families, external agencies and employees relating to the service being delivered
- To represent the Organisations philosophies within meetings, training etc
- To ensure the hours provided are in line with the contracted hours.
- To ensure the Organisation is meeting the contractual obligations provided to those Young People within our service
- You will be responsible for engaging and supporting young people who are hard to reach and providing the platform to gain employment.
- You will be expected to work with local providers, schools, Local Authorities, and other relevant agencies to support young people who have or are entering post 16 and have difficulties accessing mainstream education.
- Education and Health Care Plan (EHCP). To work with those young people with an EHCP, the company will support interventions described on the plan and look to how best to address them. The company will work closely with the Local Authority 0-25 SEND Teams, Special Education needs Coordinators and other professionals to ensure the most positive outcome.
- You will be expected to support schools, colleges and other referring bodies in the completion of their Gatsby benchmark audit and will support good practice in providing careers information, advice and guidance.
- To assist in the completion of grants and awards that will support projects and collaborations with other not for profit bodies to enhance the aims and objectives of the company.
- Be aware of, and comply with all Policies and Procedures paying specific regard to those relating to: Health & Safety; Security; GDPR and Confidentiality - reporting all concerns to the Service Lead for Education
- To ensure the organisation and those we work with remain compliant to all relevant Safeguarding Policies and Legislation
- Be aware of, and support protected characteristics, and ensure all students have equal access to opportunities to learn and develop
- Use as an interactive teaching strategy.
- Adopt a model of trauma informed and trauma sensitive strategies / approaches.
- Appreciate and support the role of other professionals and maintain positive working relationships.
- Attend relevant meetings as required
- Participate in and delivery of training, learning activities, performance development, and maintain record of own professional development.
- The post holder will be required to undertake learning / further training as directed by their line manager.
- Prepare and share reports (termly or as required) on the students educational, social, and emotional development.
- The post holder may at times be required to attend meetings / training outside of their contracted hours.
- The post holder may be required to work in other areas of the organisation’s provision.
**Experience & Job Knowledge**
- Must have experience of working with young people.
- Must have a good understanding of the legal frameworks to meet the relevant legislations within your sector - including safeguarding
- Post holder will be required to solve varied problems, such as those arising from direct learning, adapting the teaching to meet the students’ needs as well as the care and support of the students. Under the direction of the Post 16 Development Lead the post holder will be contributing to work and lessons plans up to several weeks ahead.
