Come and see what our day hospitals support workers do in our Older Persons Community Mental Health Services within Somerset Foundation Trust.

We have two Day Hospital units in the Mendip Neighbourhood that provide prescribed intervention such as Living well with Dementia, Emotional Resilience and Cognitive Stimulation Therapy.

You will be working in a multi-disciplinary team and will be expected to travel to various locations within Mendip, so it is essential you have a driving licence and access to a vehicle.

The post holder is a member of the multi-disciplinary Community Mental Health Team based in the Day Hospital in Chantry, Frome but travel to Wells and surrounding areas for group sessions. They will work in the Day Hospitals and potentially in the CMHT and Wellbeing Clinic at times, offering activities tailored to the needs of older people who experience varying degrees of memory and mental health problems, in order to improve their quality of life, maintain their existing skills and provide carer support.

Using Cognitive Stimulation Therapy, the activities undertaken are person centred to meet the needs of the client. In all cases, a warm, sensitive and stimulating environment needs to be created and you will need to demonstrate the ability to work as part of a team. Flexibility is a key quality, and you must be able to respond to clients in an appropriate manner. You will have good communication skills relating with clients and the carers and be able to demonstrate an ability to understand their needs and where possible, be able to help the carers. You will be required, at times, to visit clients in their homes so it is essential you have a driving licence and access to a vehicle. You will also have some specific administration responsibilities regarding the day hospital therefore IT skills would be an advantage.

- Maintains open communication with other members of staff within the team.
- Maintains good relationships with colleagues within the trust, Local Authority, clients, their relatives and the general public.
- Liaises with internal and external agencies, statutory, non-statutory, voluntary agencies.
- Participates and contributes in client reviews and care planning reviews with other professionals.
- As well as relating to older people at the day hospital and CMHT, you will, at times, need to be able to demonstrate an ability to understand their needs and where possible, be able to help the carers.

Assists Day Hospital Coordinator with planning of activities and other day to day tasks at the day hospital. Assists with running of day hospital sessions and managing flow of patients into and out of the building.
