Job Purpose To ensure the accurate recording, coding, summarising, and management of patient data within the clinical system SystmOne, supporting safe and effective clinical care and contributing to the achievement of NHS and practice performance targets such as QOF and Enhanced Services. Key Duties and ResponsibilitiesClinical Data Entry and Coding Read, code, and process incoming clinical correspondence such as hospital letters, discharge summaries, and test results.Ensure clinical coding is accurate, up-to-date, and compliant with current NHS coding frameworks (e.g. SNOMED CT).Summarise incoming patient notes in line with practice protocol. Data Quality and Reporting Maintain accurate patient registers for chronic disease management and QOF indicators.Run reports and data searches to support audits, recalls, and performance monitoring.Assist the clinical team with data entry for specific health campaigns (e.g.
flu clinics, NHS Health Checks). System Administration Update and maintain patient demographic and clinical information using the practices clinical system.Assist in the set-up and running of clinical templates and recall systems.Support electronic document workflow and ensure documents are allocated to the appropriate clinician. Liaison and Communication Work closely with clinicians and administrative staff to clarify coding or clinical queries.Liaise with external organisations (e.g. NHS Trusts, labs) regarding document queries or missing data.Support clinicians with the digital management of clinical correspondence.
Confidentiality and Governance Ensure all data is handled in accordance with GDPR, NHS Information Governance, and practice protocols.Report data discrepancies or concerns to the Practice Manager or clinical lead.Person SpecificationEssential: Previous experience working in a data entry or administrative role in a healthcare setting.Working knowledge of clinical systems (e.g. EMIS Web, SystmOne).Understanding of medical terminology and clinical coding systems (e.g. SNOMED CT, Read codes).Strong attention to detail and high standards of data accuracy.Ability to prioritise workload and work independently as well as part of a team.Excellent communication and IT skills.
