Elliott Hall Medical Centre is looking for Care Navigators to join our busy, friendly team, for 16.5 - 37.5 hours per week. Full training will be given and the ability to work flexible hours is essential.

Responsibilities will include the following but not exclusively:
Use computerised appointment system to book routine/emergency appointments for patients at reception or via telephone

Greet patients and deal with their enquiries patiently and efficiently, including requests for home visits

Issue and distribute repeat prescriptions both paper and electronic

Telephone/contact patients and health care professionals where necessary

Take and record appropriate messages accurately on computer system

Undertake administration tasks as required by clinicians

Manage referrals (via Electronic Referral System) including Urgent and Two Week Wait referral

Deal with referral queries from patients and clinicians

Use of scanning equipment and software

Handling patient information in line with Practice protocol

Administration support and cover

Manage the appointments and rotas

Keep patient information platforms up to date

Any other relevant duties to ensure the smooth running of the practice
