**Main Purpose of the Job**
SASP are looking for a part-time administrator to join our busy team. You will be supporting programmes aimed at adults and those living with long term health conditions in Somerset, including two brand new services. Move 2 Independence uses physical activity to help people regain their independence following a stay in hospital and Sloppy Slippers which is a falls prevention programme.
- You will be providing general administrative support to the team as required, to assist in the day-to-day delivery of their programmes - responding to general enquiries, assisting with data collection and updating activity schedules.
**Summary of Responsibilities and Key Duties of the Job**
- Receive and process referrals into Move 2 Independence. Make contact with individuals and/or their families referred into the service; complete the initial screening questionnaire and get them booked in using the online portal.
- Monitor the portal to check data inputting and run reports to capture data.
- Complete follow up questionnaires to capture feedback from individuals accessing Move 2 Independence.
- Be the first point of contact for the Sloppy Slipper falls prevention campaign. Sloppy Slippers uses pairs of proper fitting slippers to support falls prevention for people at home or in care settings. The post holder will receive and process enquiries, along with slipper requests as applicable. Administrator for Health
- Support the Active Ageing Project Officer to ensure that the recording and monitoring of resources is kept up to date and assist with the distribution of these where necessary - sending out and recording to keep track of stock and ensuring evaluations forms are being completed. Chasing up when necessary.
- Support the Walks Co-ordinator with new walk schedules or updating existing ones to ensure all records are current for members of the public to access.
- Printing Somerset Health Walks resources and sending out to walk leaders as required.
- Supporting the Physical Activity on Referral Manager by directing professionals to the referral process for ProActive. Making phone calls to new ProActive referrals, discussing their chosen provider and processing the client record on the ProActive database.
- Assist the Physical Activity on Referral Manager with distributing training webinar information to ProActive providers and NHS staff. Forwarding slides and issuing certificates upon completion.
- Assist the Active Ageing Team to provide administrative support in the day to day running of the business.
- To perform any other duties as reasonably required.
**General Information**
All work performed/duties undertaken must be carried out in accordance with relevant SASP policies and procedures, within legislation, and with regard to the needs of our customers and the diverse community we serve.
**Job Types**: Part-time, Fixed term contract
Contract length: 36 months
Part-time hours: 25 per week
**Salary**: From £21,000.00 per year
