**Housing Officer (Temporary)**

**Key Responsibilities**
**As a Housing Officer, you will**:
Identify patients ready for hospital discharge who face housing or benefit-related challenges.
Conduct comprehensive assessments of housing needs and homelessness risks.
Develop tailored plans to ensure safe and secure transitions for patients.
Liaise with local housing authorities, associations, and other key agencies to secure housing options.
Maintain detailed records and share critical updates with social workers and housing services.
**What We’re Looking For**
**We’re looking for someone who**:
Has a strong understanding of the benefits system and stays updated on relevant changes.
Brings previous experience working with individuals facing homelessness and housing challenges.
Understands the hospital discharge process and its complexities.
Has a proven ability to support individuals with mental health conditions.
Demonstrates excellent communication, organizational, and interpersonal skills.
