The role of the In-Reach Team is to improve the use of bed-based rehab services across East & North Hertfordshire. This is a multi-disciplinary team made up of clinicians and administrators and works alongside the Transfer of Care Hub (TOCH)

The team will report daily on bed status and consequently, the role will be crucial to the flow of patients from secondary care to community beds and onwards. The Team Lead will manage the team of clinicians and administrators.

The In-Reach team will oversee the use of all community beds. This includes intermediate care beds, neurological beds, enablement beds, D2A beds and short-term spot-purchased beds made by health and social care. The scope of the team will also widen to include other ‘types’ of bed should the system prove effective. It will also have strong links to the DH2A Service and an effective working relationship with Adults Social Care within the TOCH.

Operational Delivery

To act as central point of communication, information and expert advice on the rehab pathway (and other community pathways as necessary) for all

Leading the clinical screening of all referrals coming into the TOCH service

Enable safe and efficient transfer of patients into HCT bed bases by ensuring close relationships with all system partners.

Liaising with Senior HCT management team to escalate areas of concern.

Deputising for Clinical Pathway Lead as required including representing HCT on daily system conference calls, and daily assurance calls with inpatient units to support patient flow.

To proactively identify appropriate patients for bed based and home-based community care and rehabilitation.

Evaluating pathways throughout the service and lead and contribute to any service improvement projects.

Support and lead the team in their day to day working.

Supervise the team by making sure all appraisals are up to date and all performance targets are achieved.

The In Reach Team (IRT) are a small and friendly team comprised of Clinicians and Admin staff.
