The Strengthening Families service works alongside families to identify health needs, offer support, implement a plan of care, signpost to other services and deliver the pre-school Healthy Child Programme.
The service is provided to families with children aged 0-19yrs who fit into the following categories - Child protection plan, Child in Need, Children in Care and young people open to the Youth Offending Service.
Due to the complex and challenging nature of the Strengthening Families work, we pride ourselves in offering a high level of formal and informal support and supervision.
The Strengthening Families team work with some of the most vulnerable families in the community. Their main priorities are to deliver the healthy child programme and to complete health assessments to identify any unmet health needs and then to support the family to address them.

- the ability to work independently and liaise with health professionals, families and other agencies including the Local Authority Children's Services
