To work in the established Home Treatment team to provide clozapine initiation in the community.? To ensure people initiated on clozapine receive appropriate monitoring as per the clozapine protocol and other service related protocols? To facilitate and monitor the initiation of clozapine? To monitor the outcomes of clozapine initiation in community? To work with the MDT to identify people suitable for clozapine initiation in community.? To work where required within the home treatment team which would include:? intensive home support for individuals referred who are experiencing a mental health crisis.? To assess, risk manage and provide interventions for people presenting in a mental health crisis.? To facilitate supported discharge from the inpatient unit.? To appropriately refer on and signpost all patients on discharge where indicated who need assessment and/or follow up from mental health services.? To gatekeep all referrals for inpatient admission.? To work collaboratively with other agencies/mental health services.
