We are seeking a dedicated and organized Out of Hours (OOH) Care Coordinator to join our team in Durham. The OOH Care Coordinator will be responsible for managing and coordinating care services during weekends, ensuring that our clients receive continuous and reliable support.
**Key Responsibilities**:
- Coordinate and manage care services during weekends.
- Respond to emergency calls and provide immediate assistance as needed.
- Schedule and allocate care staff to meet client needs.
- Maintain accurate records of all care activities and communications.
- Liaise with clients, families, and healthcare professionals to ensure seamless care delivery.
- Provide support and guidance to care staff during out-of-hours shifts.
**Requirements**:
- Previous experience in a care coordination or similar role.
- Excellent organizational and communication skills.
- Ability to work independently and make decisions under pressure.
- Compassionate and empathetic approach to client care.
- Proficiency in using care management software and other relevant tools.
- Availability to work weekends.
