We are looking for a dedicated and experienced **Team Leader**to join our management team and help maintain these high standards.

**Position Purpose**:
The Team Leader is a key role within our team, ensuring that care services are provided in line with individual care plans, service agreements, and the required standards.

**Key Responsibilities**:
As a Team Leader, you will:

- Conduct needs assessments and develop individual care plans for clients.
- Supervise care workers and teams, offering guidance, advice, and mentoring as required.
- Act as a point of contact for care workers seeking advice and support with any challenges.
- Provide direct care in emergency situations or out of hours when necessary.
- Conduct spot checks and report findings to management.
- Regularly visit service users to gather feedback and address any concerns or issues.
- Investigate and report on concerns, including safeguarding issues and complaints, to the service manager.
- Assist with staff induction and training, and assess new staff for their ability to work independently.
- Organise care reviews and ensure that any changes to care plans are implemented.
- Review daily notes, incidents, and concerns to ensure high-quality care.
- Provide support to the senior management team, including the Registered Manager
- Collaborate with other professionals involved in the care of clients to ensure comprehensive support.
- Contribute to the service quality improvement and business planning strategies.
- Engage in continuous professional development through relevant training and qualification
- Undertake any administrative duties as directed by your manager.
- Occasional on-call duty to cover staff absences when necessary

**Key Requirements**:

- Previous experience in a care coordination or related care role is essential.
- Strong leadership and supervisory skills.
- Excellent communication, interpersonal, and organisational skills.
- Ability to work flexibly and respond effectively to emergencies.
- Commitment to continuous professional development.
- Knowledge of the Health and Care Act 2012, CQC standards, and national guidelines.

Pay: £29,000.00 per year
