Stroke Rehabilitation Assistant Practitioner

An exciting opportunity has arisen for Stroke Rehabilitation Assistant Practitioners to join our Stroke team. We are looking for an enthusiastic and highly motivated individuals with good communication, organisation and interpersonal skills, with the ability to work as part of a multi-disciplinary team.
You will be supported by Physiotherapists, Speech and Language therapists Occupational therapists and a dietician. The role involves working within the stroke team across Swansea Bay, in the assessment and rehabilitation of patients both in hospital and community settings.
You will need to be able to demonstrate significant experience of working in rehabilitation in NHS, Social Care or Community based settings. Previous therapy assistant experience and experience of working with individuals who have had a stroke is highly desirable for this role.
The post holders must be able to work with discretion and maintain confidentiality at all times.
To support Physiotherapists, Speech and Language therapists, Occupational therapists and dietetics working within the stroke team across Swansea Bay, in the assessment and rehabilitation of patients.
The work will be varied and based both in hospital and community settings, so the ability to travel independently is essential for this role. The role involves therapeutic interventions and visits to aid prompt discharge, and assessing patient progress in their home. The post holder will be required to work within any of the in-patient stroke units to promote rehabilitation, enhance outcomes and aid early discharge.
The post holder is required to work unsupervised, undertaking independent interventions and decision making within agreed protocols in the community and on the wards, although some supervision and support would be available from qualified therapists.
The role may involve the fitting and instruction on use of adaptive equipment, the supervision of adaptive daily living skills and leading therapeutic groups. The post holder may be required to carry out any of these tasks single-handed.
To assess and undertake agreed treatment programmes with individual clients/patients.
