An exciting opportunity has arisen for a full-time Band 4 Pathway Coordinators to join the Woodfield Child Development Service, The Medical Centre, Woodfield Road. We are seeking two highly motivated and enthusiastic Pathway Coordinator who are looking to develop their skills in the largest community healthcare trust in London.

The Woodfield Child Development Service is run in partnership between CLCH and Imperial College Healthcare NHS Trust. We are committed to providing high quality care to the local community. We offer a wide range of services for children and their families, involving multidisciplinary group of experts from the health, social and educational sectors.

The coordinator’s role is pivotal to the overall delivery of the service ensuring that patients’ pathway of care/assessment from referral to discharge is managed within agreed timescales.

Effective time management and communication are key to the success of this role. The coordinator will be liaising with both patients, clinicians as well as external agencies and stakeholders. The post holder will coordinate/support patients through the pathway, ensuring appointment bookings and waiting list are managed effectively. Demonstrating effective use of clinic capacity and help ensure the Trust meets local and national waiting time targets..

Excellent inter-personal, written and verbal communication skills with clear understanding of issues of confidentiality are required. The ability to deal in a calm, approachable way with patients, staff and external agencies in order to manage all enquiries effectively and sensitively is vital.

Coordinate all administrative processes for each patient’s pathway to ensure it is managed smoothly and within agreed time frames

Ensure all patient demographics and contact details are kept up to date on Systmone/Cerner

Phone parents of children on the pathway to provide instructions, reminders and information where necessary.

Attend and participate in meetings relevant to the role as required.

Take and transcribe minutes of meeting in a timely manner when appropriate.
