This is a real opportunity to be part of a service alliance that works closely together to deliver coordinated mental health services.The Mental Health Pathway Co-Ordinator will have responsibility for screening coordinating and facilitating moves into and across and out of services working closely with Providers Mental Health operational health/social care staff Housing and Commissioning. The post will manage flow into and out of the pathway including identifying and managing voids; tracking service user progress towards move-on whilst identifying any barriers to move-on and Provider issues finding solutions in partnership with Providers and relevant partners.
