- (5 over 7 day rota to cover evenings, weekends, bank holidays and outreach support during the day to meet service provision)_

**Experience**:
- Proven experience working with people experiencing emotional or mental distress
- Experience in undertaking day to day operational duties
**Summary**:
As part of Mid and South Essex Sustainability and Transformation Partnership (STP), there is a range of provision to provide timely and appropriate responses to people in crisis to prevent escalation of mental health problems and unnecessary referrals to specialist/complex care mental health services, Accident & Emergency (A&E) and other emergency and out-of-hours services, and to provide support in the community to reduce the inpatient stay.
The Admission Prevention & Early Discharge Service, the NHS Home Treatment Teams (HTT), Crisis Resolution Service (CRS), Mental Health Liaison and Sanctuary Plus Crisis Housing will provide a joint service.
The Admission Prevention element will provide extra support to people in crisis that can remain at home as an alternative to admission.
Following an inpatient experience in the Mental Health Unit, the Early Discharge element of the service will support people to settle back into their home environment and community.
This is a 7 day a week service between the hours of 7.00am and midnight.
**Job Purpose**:
To support adults experiencing emotional and mental distress in South East Essex who have been referred by Community Mental Health Team (CMHT), Primary Care Network (PCN), Home Treatment Teams (HTT), Mental Health Liaison, Crisis Response Service (CRS) or Crisis Sanctuary.
Using a person-centred, strengths-based approach to ensure people receive the practical and emotional support to enable them to live independently in a lifestyle of their choice.
**Main Duties and Responsibilities**:
**Communication**
- To provide person centred support to service user’s experiencing emotional or mental distress by telephone, digital platforms or in person.
- To positively promote the understanding that the service user is in control of the pace of their recovery journey and its route.
- To communicate with people in a manner that is consistent with their level of understanding, culture, and background so that they can make informed choices about their recovery and the options available.
- To develop effective, positive, and respectful working relationships with service users, carers, and professionals.
- To work in partnership with the NHS, Social Care and related community services to deliver a personalised programme of care that will meet the needs of the service user.
**Interventions**
- To manage a caseload of service users, including people with complex needs and long-term conditions.
- To receive and efficiently process referrals
- To give service users time to tell their stories and focus on ‘what matters to me’. Build trust with the person, providing non-judgmental support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a person’s assets.
- To help service users identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
- To ensure all relevant information about service user’s support or level of risk that gives cause for concern is escalated to the Manager.
- To work with service users to co-produce a simple personalised wellbeing plan - based on the person’s priorities, interests, values and motivations - including what they can expect from the groups, activities and services they are being connected to and what the person can do for themselves to improve their health and wellbeing.
- To deliver approaches which engage with service users in a boundaried manner, to enable individuals to access an appropriate range of support.
- To signpost service users and / or facilitate referrals to community-based services and activities, including peer support, health, social, leisure, employment and training opportunities.
- To offer practical help and guidance to the service user to enable them to sustain their own physical and mental health, educational and social wellbeing.
- To support the service user with practical and domestic tasks to enable them to maintain a habitable home environment.
- To ensure the service user has help and guidance with their own money in order to pay bills, rent and other general living expense.
