**About Us**:

- Caring Connections is a registered charity with over 30 years’ experience of supporting the Merseyside area.
- As a charity we specialise in providing services to improve people’s physical and emotional wellbeing alongside reducing social isolation.
- Our services include CQC registered home care, local heritage projects, suicide prevention programmes and community support schemes.
- Aim of project to support homeless individuals who have been admitted to hospital to move foreward within their own home and / or hospital accomodation.

**Responsibilities**:
**(Individuals must have a valid UK driving Licence and access to their own vehicles)**
- To provide personalised reablement support and care to homeless individuals across YMCA supported accomondation properties.
- To work to maximise an individuals levels of independence.
- To follow care plans to support individuals to improve their physical / emotional wellbeing.
- To deliver personal care (when appropriate) to individuals promoting dignity and respect throughout.
- To travel across a set area to work with a number of service users per day.
- To work in alignment with a support team to provide the highest quality of care experience.

**Requirements**:

- No experience required, the most important is your values as a person. If you are a caring person who is looking to make a difference please apply.
- As travel is an important part of role it is important that you drive and have access to your own vehicle.
- If you have worked in care before there are a number of opportunities in senior care roles.
