To provide clinical leadership for the Home First Hub and support the wider community health and social care teams ensuring that safe and effective quality care is delivered. Secure and assure the highest standards of clinical care by the provision of clinical leadership and support for staff delivering clinical services, service users and their relatives.
1. To provide effective leadership and be responsible and accountable for the operational management, co-ordination and delivery of Home First services Home First Acute flow, Home First Hub and Home first Rehab & Reablement.
2. You will lead a multi-professional within SCO in accordance with set objectives, local and national targets, quality standards, controls and resource constraints that ensure the delivery of high - quality patient centered services that meet patient needs.
3. To be a budget holder and be responsible and accountable for managing the budget within given resources.
4. To ensure all care is delivered according to Trust policies and procedures ensuring appropriate governance arrangements.
5. To utilise clinical audit, research, supervision and teaching to monitor and improve standards of care by efficient and effective use of resources.
6. Your multi-disciplinary team made up of AHPs,RNs, Soical Work, Administration Staff, Voluntary Sector colleagues and Housing all involved in supporting people to return home after their discharge. The Home First Hub is centered around why not home, why not today. We want to reduce the risk of harm to the person by delays in their transfer of care and we want to improve people's outcomes, home is best.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months.
