**JOB TITLE: Family Support Worker**

**ACCOUNTABLE TO: Family Support Services Manager/ Supervisor**

« To adhere to all organisational Policies and Procedures in respect of carrying out support and Care Duties including and in other administrative aspects of the service as required.

« Support young people to develop and maintain safe social networks with peers.

« Ensure that the child/young person(s) is aware of the local community resources available to them.

« Ensure the physical environment for the child/young person(s) enables them to lead a safe and healthy life.

« Where necessary, offer support in relation to emotional, social and financial development, for the child/young person.

« Ensure that child(ren)/young people are aware of and are motivated to seek and can access or receive various statutory or community based project entitlements/resources.

« Befriend and mentor the young person and/or where necessary encourage them to participate in their preferred recreational and leisure activities.

« Where appropriate encourage the child’s/young person’s to maintain links and continued contact with their immediate family and extended family members.

« Encourage and support young people in developing and maintaining training/education/career interests.

« Assist and attend meetings in relation to the children/young people in accordance with their identified educational needs. Act as a representative of IFS on behalf of the child(ren)/young people in relation to their educational development and progression.

« To provide discreet supportive monitoring of the young people’s self-care. Encourage each young person’ in the management of personal care and where necessary offer training/guidance and information on elements of personal-care.

« To provide personal care where required which may involve assistance with; Continence requirements; Health and Medication requirements; Manual Handling; eating and preparing meals. The service users dignity, privacy and independence must be maintained as far as practical:
« To provide a provision of specialised care to service users with specific identified needs according to your own skills and qualifications.

« Maintain accurate written records of all interaction with young people.

« In conjunction with the Case Worker/Manager, make sure that all development plans and care plans are completed and regularly evaluated.

« As directed by the Case Worker/Manager, attend any meetings related to the outreach support of the young person(s).

« Report the outcome of each visit to the Manager or designated person.

« To be part of a rota system, to provide emergency and on call service(s) to children/young people.

« To accompany young people to and from school and other appointments as required.

« To report all accidents or injuries, whether it be to yourself or the Service User. In accordance with IFS policies and procedures

« To carry out any other duties responsibilities as required by the Manager.

**Job Type**: Permanent

**Salary**: £10.00-£13.71 per hour

Day range:

- Every weekend
- Monday to Friday

Shift:

- Day shift
- Night shift
- Overnight shift

**Experience**:

- work: 1 year (preferred)

Licence/Certification:

- Driving Licence (preferred)

Work authorisation:

- United Kingdom (required)

Work Location: On the road
