What youll do: Under the guidance of registered nurses in the team, assess and monitor changes in a patients condition, reporting all physical, social, psychological and spiritual needs Promote and maintain comfort, dignity and respect to all patients, carers and colleagues Establish and maintain effective communication channels between patients, families and healthcare professionals Under guidance from registered nurses, carry out personal care of patients, updating care plans as appropriate Undertake clinical tasks under direction of registered nurses, and within assessed competencies Identify, assess and report risks to registered nurses Attend mandatory training events and ensure professional knowledge is updated Promote the hospices values within the Hospice at Home team and your day-to-day work Adhere to hospice and community guidelines and policies to ensure best practice
