**Family Support Officer / Vision Impairment SEND Support Worker** with experience of hospital outreach meetings and an NVQ Level 3 (or equivalent) in a relevant field, or demonstrable relevant experience is required by the Children & Young People Services team at a well-established Charity.

**SALARY**:£28,988 - £35,199 per annum

**JOB OVERVIEW**

We have a fantastic new job opportunity for a Family Support Officer / Vision Impairment SEND Support Workerwith experience of hospital outreach meetings and an NVQ Level 3 (or equivalent) in a relevant field, or demonstrable relevant experience.

As the Family Support Officer / Vision Impairment SEND Support Worker you will provide an information, advice and guidance service to parents, carers and families of Children and Young People with a vision impairment.

The Family Support Officer / Vision Impairment SEND Support Worker helps families appropriately, empowering them to access support and services that will assist them to realise their required outcomes.

You will work part of the week in London based clinics supporting families and liaising with the Charity’s and other CYP Services where there is direct involvement in the same family.

**DUTIES**

Your duties and responsibilities as the Family Support Officer / Vision Impairment SEND Support Worker will include:

- Offer allocated families high quality information, advice, and guidance, including at and around initial diagnosis
- Work proactively with enquiring families, CYP and partner agencies, to empower parents to support their child/ren and empower CYP to self-advocate
- Produce resources for service-users (e.g. self-help guides) that empower them to support themselves
- Collate and capture relevant information on the CRM database
- Manage data within the team and the wider CYP Services in line with General Data Protection Regulation (GDPR) regulations
- Take responsibility for collecting and disseminating relevant information
- Develop strong links with other teams and staff to ensure families receive a holistic and joined-up service that is person-centred and meets their needs
- Develop strong links with key agencies to ensure the organisation can be the first port of call for families at the time of diagnosis, and thereafter
- Develop understanding of other networks nationally to support the IAG on offer to families and to enhance the onward referral experience for families
- Maintain a current and good understanding of the full range of CYP services available
- Contribute to the collation, analysis and reporting of data, statistics, reports, support programme documentation and activity records relating to caseload
- Attending events, meetings and promoting the service/s

**CANDIDATE REQUIREMENTS**
- Educated to degree level or equivalent (QCF level 6 / SQCF levels 9-10)
- Experience of providing IAG to families with disabilities
- Awareness of relevant policies and legislation in relation to Health & Social Care, Special Educational Needs and Disability (SEND)
- Working knowledge of vision impairment and/or disability
- Knowledge of the welfare system in relation to children with disabilities
- Assessment and planning skills
- Effective verbal and written communication skills, including presentation skills
- Proven organisational, time-management and administration skills
- Ability to process and record data, respecting GDPR regulations
- Competency in ICT, including Office packages and database administration
- Eligibility to work in the UK
