We are seeking an enthusiastic and self-motivated Track Lead to join our dynamic team.

The post holder will be required to effectively manage a workforce of admin staff to assist in the delivery of safe and effective patient transfer and tracking within the Discharge to Assess Service.

The role will require an understanding of the logging, tracking and trending of incidents/problems/changes in the live environment of programmes used by the teams.

The post holder will build and maintain strong relationships with all partners and members of the clinical triage team to support discharge and admission avoidance as the service dictates.

The Track Lead - Track and Triage; is required to effectively manage a workforce of admin staff to assist in the delivery of safe and effective patient transfer and tracking within Discharge to Assess Services, provide Local Health Economy information flows and service assurance.

Responsible for information flow to the local health economy, and for the Track Function of the patient journey.

Analysis and validation to report on deficit in the system and provide senior level escalation

The role will require an understanding of the logging, tracking and trending of incidents / problems / changes in the live environment of programmes used by the teams. Close working with Local Health Economy partners on reporting will form part of the role and information flows to partners and MPFT managers will be a key part of the function of the team

The role will involve close working with health and social care staff within the triage function and an understanding of community Home First services will be essential alongside a desirable knowledge of the Care Act 2014

The post holder is expected to work closely with the Track team, Clinical Leaders and Integrated Service Manager to prioritise workload/work packages to ensure timely and quality deliverables.

The role will be required to assist in work planning, facilitate support requirements of the team, log incidents and monitor progress of the system flow.

The post will be providing a 7 day a week service operating from 8am to 8pm, the post holder will be expected to work on a rota basis giving coverage across these hours 7 days a week with potential to be part of an on call rota.

The Track Lead will be expected to step up to provide operational lead cover alongside and in the absence of the Service Manager/Operational Lead.

The Track lead will manage staff supervision, training monitoring and appraisals. Complete working rotas for staff and manage sickness and absence
