Job Introduction
As a Healthy Family Practitioner you’ll join a team who are focused on having a positive impact on children and families by supporting those in your local community through facilitating parenting groups and offering 1:1 guidance to really make our service users and staff feel the difference.
Main Responsibilities
As a Healthy Family Support Worker within HCRG Care Group you’ll join a team who are focused on clinical excellence and positive outcomes for children and families.
Your days will be rewarding and varied and include:
- Working across the Lancashire Young People & Healthy Families Service with preschool and primary school aged children and their families under the direction of Health Visitor, School Nurse and Staff Nurses.
- Utilising your knowledge of child developmental milestones to provide key elements of the Healthy Child Programme.
- Targeted support based on need as delegated by a Specialist Community Public Health Nurse / Team Leader and members of the wider team.
- You will predominantly work within the School Nursing team supporting the delivery of the National, Child Measurement Programme (NCMP), School Health Needs Assessment and other targeted work related to children and families.
You will have a good general education and evidence of personal and professional development. You will have experience of working in partnership with families and liaising with parents regarding their child’s needs. Most importantly we are looking for someone who can work as part of an effective team and is passionate about the wellbeing and development of children in the local area.

Package Description
As an **Apprentice Healthy Family Support Worker**, you will complete a nationally recognised **Level 3 senior healthcare support worker qualification**, with diploma in healthcare awarded through City and Guilds. To support you in successfully completing your programme you’ll be given **monthly learning days with weekly study time to develop your knowledge and skills.**
A **dedicated assessor** who will support you in your journey and help you realise and achieve your career aspirations. A work mentor who works in your department to support you through your apprenticeship and settle you into your new role.
