Mr K has a diagnosis of Paranoid Schizophrenia and was in care for a long period as a child. Before he came to MH Reablement Mr K had frequent re-admissions to hospital, his risks included social isolation and self
- neglect.
Our Independent Living Assessor/ Occupational Therapy Care Manager (OTCM) of the MH Reablement Team worked closely with MR K to support him with his activities of daily living and he became independent in managing laundry, personal care tasks and completing meal preparation independently. We improved his health and nutrition, reduced hospital admissions and he met his re-ablement goals and continues to flourish independently within our community.

**The Role**:
As the Mental Health Reablement Team’s Independent Living Assessor you can make your own powerful contribution as you help coordinate support, set goals and review the progress patients are making, and ensure that they’re able to live as self-sufficiently and safely as possible while enjoying a positive day-to-day life. Your work will focus on helping patients regain and maintain their independence, which will in turn reduce the need for long-term support.

Assessing, coordinating and monitoring each patient’s Mental Health Reablement programme and regularly reviewing their progress towards the attainment of set goals, you’ll arrange for the provision and adaption of specialist equipment, and occasionally its fitting. Processing incoming referrals, and monitoring and reviewing cases to ensure an appropriate service is in place, you’ll also develop support plans for patients who have ongoing care needs that are below an agreed financial threshold.

You’ll have a wide range of additional responsibilities, including completing a discharge assessment for each patient on conclusion of their Mental Health programme, and completing referrals and reports which meet their service needs. Liaising on behalf of patients with other departments, NHS staff, agencies and organisations in order to provide a quality reablement service, you’ll also be ready to deal with patient enquires regarding the services provided by our Mental Health Reablement Service and Community Teams.

**About You**:
Our Mental Health Reablement Team delivers an integrated, people-centred service that focuses on strength and rehabilitation, and with this in mind you’ll understand the importance of care coordination and the role of assessor within health and social care. And with excellent communication skills and the vision and drive to help establish this new service, you’ll embody our values of respect, integrity, collaboration and inclusiveness.

We’ll expect you to have GCSEs or their equivalent in both Maths and English, a Diploma or NVQ Level 3 in Health and Social Care or a related field, basic Microsoft Office skills, and an understanding of the conditions that can lead to disability and their impact on individuals. It’s important that you possess knowledge of the range of services available to people with disabilities, and in addition to familiarity with anti-discriminatory practices and the needs of diverse communities, you’ll also be aware of the importance of the roles of Occupational Therapy and Physiotherapy in communities.

Capable of working cooperatively in a multi-disciplinary team and alongside a service user’s family and support networks, and willing to undertake on-going professional learning and development, you’ll have the ability to prioritise your tasks and organise time effectively. We’ll also look for the skills to assess users’ needs, develop goal directed reablement programmes, monitor progress, modify plans as appropriate, recommend appropriate equipment and adaptation work, and implement set procedures.
