This is an exciting opportunity for a professional to enhance their clinical and communication skills working across health and social care settings to ensure the best provision of care to patients requiring complex care from SCAS services in 111 and 999. The post holder will need to have empathy towards patients with complex needs who have challenging behaviours, confusion or have difficulty understanding.

The main aim is to gain an understanding of the root cause of the of calls in order to align the patients care with the most appropriate provider. The post holder will need to be able to work with other professionals as an ambassador for the service and the patients in their case load, to agree safe provision of care for these complex patients acknowledging the pressures that other services are potentially facing.

The aim of the joint working is to produce, review and maintain a patient management plan for all the patients in your care that meets the needs of all stakeholders, putting the patient at the centre of the plan. The patient management plan will also need to include a risk assessment to ensure the safety of the care providers where complex patients may pose a risk due to displayed behaviours or in times of crisis.

About Us

South Central Ambulance Service NHS Foundation Trust provides a range of emergency, urgent care and non-emergency healthcare services, along with commercial logistics services.

The Trust delivers most of these services to the populations of Berkshire, Buckinghamshire, Hampshire and Oxfordshire as well as non-emergency patient transport services in Surrey and Sussex.

MAIN DUTIES AND RESPONSIBILITIES

Patient and Professional Focused

The Complex Care Practitioner role always requires the highest standards of professionalism. You will work with a variety of complex patients, partners, providers and professionals. You are an Ambassador for SCAS

To provide a high standard of patient care and support to Very High Intensity Users (VHIU), aligned to the role

To work effectively with a diverse range of professionals from many specialisms such as GP’s, District Nursing teams, Mental Health team’s and Social Care.

To maintain high quality and accurate records of discussions, meetings and case conferences with patients, professionals and premises

To coordinate and ‘chair’ case conferences, multi-disciplinary meetings and management meetings appropriate to need to the Complex Care Lead any internal actions or recommendations.

To analyse clinical records and data to determine root causes of activity

To coordinate and lead on the individual assessment, review and management of Very High Intensity User (VHIU) patients

To coordinate and lead on the partnership support of VHIU premises or professionals, proposing solutions and new ways of working to reduce the root cause of emergency demand (align patients to appropriate care, reduce risks for patients, health promotion)

To direct professionals on likely referral routes for individual patients, appropriate to need and patient’s situations such as fire risks to the fire service and violence and aggression risk to the Health and Safety Team

To put in place all the elements (safety, welfare) of PMPs for patients

To write/review Patient Management Plans (PMP) appropriate to need

To determine the process of wrap around and welfare for patients managed with a PMP

To write Premises SOPs aligned to SCAS and premises need

To review Very High Intensity User (VHIU) patients who are being managed with a PMP

To maintain a high level of security of patient’s confidential information

To complete all necessary documentation and reporting, including patient records, in accordance with Trust procedures.

To maintain the appropriate care of equipment, vehicles, uniform and any other equipment provided by the Service, reporting any defects immediately.

To adopt a flexible work pattern.

To work closely with the safeguarding Team to coordinate intelligence and inform safe planning

To work closely with the Mental Health and Learning Disability Lead to coordinate intelligence and joint planning to support the patient’s mental health needs.

To provide upward reports to the Complex Care
