Job Summary

A dynamic, patient-focused healthcare organisation is seeking an Advanced Nurse Practitioner (ANP) to join its multidisciplinary primary care team. Main Duties of the Role

  • Work as part of an established ANP team, alongside GPs, clinical pharmacists, practice nurses, HCAs, and other clinicians.
  • Contribute to triage and duty sessions, assessing and managing patients via telephone, face-to-face appointments, and home visits.
  • Independently assess, diagnose, and manage acute and chronic conditions for patients of all ages, including those with mental health needs.
  • Provide safe prescribing and medication reviews in line with evidence-based guidelines.
  • Request, interpret, and act upon diagnostic investigations (e.g., blood tests, scans, X-rays).
  • Coordinate care plans and make referrals to internal and external healthcare teams.
  • Deliver services to meet organisational and national clinical targets.
  • Support health promotion, disease prevention, screening programmes, and clinical education initiatives.
  • Perform a range of advanced clinical tasks including immunisations, wound care, contraception clinics, minor procedures assistance, and routine health checks.

Administrative & Professional Responsibilities

  • Maintain accurate, timely clinical records in accordance with legal and organisational requirements.
  • Participate in clinical meetings and contribute to service development.
  • Support the management of clinical areas, stock levels, documentation, and audits.
  • Assist with service planning, policy development, and nursing team coordination.

Training, Leadership & Development

  • Maintain up-to-date clinical skills and knowledge at an advanced level.
  • Participate in annual appraisals, clinical supervision, and ongoing professional development.
  • Contribute to staff education, training programmes, and mentorship of junior colleagues.
  • Provide clinical leadership, supporting safe and effective delivery of nursing services.
  • Assist with recruitment, staff appraisals, rota planning, and quality assurance processes.

Quality & Governance

The post-holder will:

  • Respond to issues of quality and risk.
  • Reflect on practice to support service improvement.
  • Work effectively within multidisciplinary teams and external agencies.
  • Manage workload efficiently and prioritise care appropriately.

Person SpecificationExperience

Essential:

  • Experience working autonomously in an advanced nursing role.
  • Ability to assess and manage patients across all age groups, including those with mental health needs.
  • Independent Non-Medical Prescriber.
  • Minimum 1 year of triage experience.

Desirable:

  • Primary care or general practice experience.
  • Case management experience.
  • Nurse-led triage experience.

Skills & Knowledge

  • Strong clinical leadership skills.
  • Excellent communication (written and verbal).
  • Ability to deliver difficult messages sensitively.
  • Negotiation, conflict resolution, and change management skills.
  • Advanced clinical assessment and examination skills.
  • Experience managing patients with long-term and complex conditions.
  • Understanding of clinical governance, NHS policy, and the wider health economy.

Desirable:

  • Ability to engage, inspire, and motivate multidisciplinary teams.
  • Experience producing reports, presentations, and written communications.

Analytical Skills

  • Ability to analyse and interpret complex information and recommend appropriate clinical actions.

  • Experience conducting clinical audits.

Qualifications

Essential:

  • Registered Nurse with relevant MSc or equivalent qualification.
  • Independent prescribing qualification.
  • Minimum 2 years' experience in triage.
  • Qualification in long-term condition management.

Desirable:

  • Teaching or mentorship qualification.

  • Role requires an enhanced Disclosure and Barring Service (DBS) check.
  • Applicants must hold valid UK professional registration.
