Key Responsibilities1. Clinical Care and Prescribing Conduct structured medication reviews (SMRs) and follow-up consultations. Prescribe independently within competence, following local formularies and NICE guidance. Support management of chronic diseases (e.g., hypertension, diabetes, asthma, COPD, cardiovascular disease).

Review repeat prescriptions for accuracy, safety, and cost-effectiveness. Monitor high-risk medications, titrate doses, and manage adverse effects. 2. Medicines Optimisation Implement evidence-based medicines management and optimisation strategies.

Undertake medicines reconciliation after hospital discharge or care transitions. Support antimicrobial stewardship and other prescribing initiatives. Identify and report medication errors or safety concerns. 3.

Collaboration and Integration Work closely with GPs, practice nurses, and other healthcare professionals. Liaise with community pharmacies, hospitals, and care homes to ensure continuity of care. Participate in multidisciplinary team meetings and PCN initiatives. 4.

Education and Leadership Provide professional support and training for practice staff and pharmacy trainees. Deliver patient education sessions, including adherence support and medicine counselling. Contribute to audits, quality improvement projects, and service development. 5.

Governance and Professional Responsibilities Maintain accurate clinical records on EMIS, or equivalent systems. Ensure compliance with GPhC professional standards, NHS clinical governance, and data protection regulations (GDPR). Engage in continuous professional development (CPD) and clinical supervision.
