We are looking for an enthusiastic and motivated Clinical Pharmacist to join our friendly and forward-thinking General Practice As part of our multidisciplinary clinical team, you will play a key role in supporting safe, effective, and evidence-based prescribing, improving patient outcomes, and reducing the workload of GPs through expert medicines management and clinical care. You will work directly with GPs, practice nurses, pharmacy technicians, and other healthcare professionals to optimise medicines use across the practice population. Your work will include structured medication reviews (SMRs), chronic disease management, repeat prescription reviews, medicines reconciliation, and supporting long-term condition pathways in line with local and national priorities. If you are an experienced pharmacist and Independent Prescriber, this is an excellent opportunity to develop your clinical skills in a supportive, innovative environment. You will be encouraged to undertake further training CPPE Primary Care Pharmacy Education Pathway (PCPEP) and to contribute to quality improvement and service development. Main duties of the job Conduct and document structured medication reviews (SMRs). Provide face-to-face and telephone consultations for medication queries and chronic disease management. Support prescribing safety, cost-effectiveness, and adherence to local formularies and NICE guidance. Lead or support medicines optimisation projects, audits, and prescribing initiatives. Collaborate closely with community pharmacies, secondary care, and other local health partners. Work within the NHS England Additional Roles Reimbursement Scheme (ARRS) framework. Clinical Care and Prescribing Conduct structured medication reviews (SMRs) and follow‑up consultations. Prescribe independently within competence, following local formularies and NICE guidance. Support management of chronic diseases (e.g., hypertension, diabetes, asthma, COPD, cardiovascular disease). Review repeat prescriptions for accuracy, safety, and cost‑effectiveness. Monitor high‑risk medications, titrate doses, and manage adverse effects. Implement evidence‑based medicines management and optimisation strategies. Undertake medicines reconciliation after hospital discharge or care transitions. Support antimicrobial stewardship and other prescribing initiatives. Liaise with community pharmacies, hospitals, and care homes to ensure continuity of care. Participate in multidisciplinary team meetings and PCN initiatives. 4. Education and Leadership Provide professional support and training for practice staff and pharmacy trainees. Deliver patient education sessions, including adherence support and medicine counselling. Contribute to audits, quality improvement projects, and service development. 5. Governance and Professional Responsibilities Maintain accurate clinical records on EMIS, or equivalent systems. Ensure compliance with GPhC professional standards, NHS clinical governance, and data protection regulations (GDPR). Engage in continuous professional development (CPD) and clinical supervision. Person Specification Qualifications Pharmacy Registration Must be a fully registered Pharmacist with the General Pharmaceutical Council (GPhC). Current, active registration without restrictions. Pharmacy Degree MPharm degree or equivalent recognised qualification. Completion of pre‑registration training (or equivalent). Independent Prescriber (IP) qualification recognised by the GPhC. Ability to prescribe safely within scope of competence. Continuing Professional Development (CPD) Evidence of ongoing CPD, keeping up to date with clinical guidelines, prescribing, and patient care. Primary Care Pharmacy Education Pathway (PCPEP) Completion or enrolment in the CPPE PCPEP or equivalent post‑registration training in primary care pharmacy. Advanced Clinical or Specialist Qualifications Clinical diplomas, MSc in Clinical Pharmacy, or other postgraduate clinical qualifications. Accredited training in long‑term condition management (e.g., diabetes, hypertension, respiratory disease). Leadership or Teaching Qualifications Certificates in supervision, mentorship, or healthcare education. Experience or training in leading quality improvement projects, audits, or service development.
