An exciting opportunity has arisen for a full-time Band 4 Associate Practitioner to work within Brent Community Mental Health. Based in the London Borough of Brent, this is a fantastic opportunity to work in a challenging and diverse role. You will be joining an experienced and friendly multi-disciplinary team in a supportive working environment and be committed to providing a high quality of service to our population.

Main duties of the job
To be considered for this post you will need to have a positive, hopeful and can do attitude. You will possess excellent communication and interpersonal skills, and an ability to work under pressure. You will have knowledge and experience of caring for people with mental illness and be passionate about mental well-being.
The post holder will be expected to work as a member of the multidisciplinary Locality Teams with a focus on enablement and out-reach work to support service users towards their recovery journey. The role will be key in supporting people with emerging and long term mental health conditions with regards to mental and physical health outcomes, social care and social inclusion.

Detailed Job Description And Main Responsibilities
The post holder will contribute to the growth and maintenance of key functions of the clinical area by carrying out assigned tasks, involving direct and indirect care to service users, under minimal supervision and as directed by the Senior Practitioners/Team Leaders.
Following personal development planning and targeted training, the competent practitioner will be in a position to work more autonomously in supporting service users. As a result you will have developed a broad underpinning knowledge at intermediate level, and have the ability to evaluate information and communicate effectively with a range of professionals.
To support members of the Brent Community Mental Health Team in ensuring a high standard of nursing and social care. To provide direct physical and psychological care to a designated group of service users through participation in the care planning, provision and evaluation of service user needs under the supervision of trained staff.
To assist in the provision of a community mental health care service to the residents of Brent.

Qualifications
Essential criteria
- To have completed Secondary Education, with pass in GCSE English (A * - C) / National Certificate in Literacy / ESOL Level 2 or equivalent.
Desirable criteria
- Any qualification/diploma related to health/social care/housing support
Experience
Essential criteria
- Experience of working as part of a team with people with acute / severe and enduring mental illness.
- To have had experience in working unsupervised, and in prioritising a workload.
Desirable criteria
- Previous experience as a support worker/associate practitioner/healthcare assistant
- Previous experience of co-ordinating aspects of care for people with acute / severe and enduring mental illness.
Skills & Knowledge
Essential criteria
- Willingness to undertake further training as required.
- A working knowledge of mental illness/mental health.
- Well developed verbal and written communication skills
