The post holder will play a key role in the delivery of Haematology Services throughout the Trust. The post holder will provide early input of specialist care to Haematology patients.
An exciting opportunity has arisen to join the Haematology Clinical Nurse Specialist team at George Eliot Hospital.
The Haematology team is a dynamic, supportive and friendly multi-disciplinary team dedicated in providing holistic, patient-centred care. We are therefore looking for a highly motivated and dynamic individual who is committed to providing the highest standard of nursing support and care to both inpatient and outpatients.
Any applicants must have knowledge and experience in haematology and chemotherapy and possess excellent communication skills. You will be working closely with the multi-disciplinary Haematology team, and therefore, must be confident in acting as the patient's advocate to ensure the best outcomes throughout their diagnostic and treatment pathway.

Managerial/Leadership
Undertake clinical supervision of nursing colleagues on an individual or group basis.
Evaluate service delivery against key performance targets, National Quality Measures, identify areas for improvement and initiate change.
Ensure the effective and efficient use of physical and financial resources, make recommendations regarding supplies and equipment.
Collect, collate and report activity data to key stakeholders, producing regular formal reports.
Embeds patient and public involvement within the sphere of practice.
Promotes the philosophy of person-centred care, co-creating care with people affected by cancer
Participates in Service Reviews in order to drive service improvement.
Leads the development of patient focused education, including training to self-manage ongoing consequences.

Principal Accountabilities
Assess, plan, implement and evaluate programme of evidence-based nursing care.
Provide expert advice and clinical practice.
Provide specialist education and training to others.
Work collaboratively and co-operatively with clinical colleagues to develop cost effective services and quality of care delivered.
Act as a role model, lead the development of clinical services within the specialist field.
Actively participate in multi-professional meetings, acting as patient advocate and representing nursing views, using creative reasoning and problem solving where necessary.
Act as a Key Worker for a clinical caseload, working to co-ordinate care and provide easy access for ongoing support for patients.
Communication
Communicate effectively with colleagues, patients and other users of the Trust.
Produce written communications which are clear, fluent, concise and easily understood
Establish and build relationships with colleagues and clients/users and the effective management of patient/client concerns in line with the Trust policy.

Innovation/Education
Provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives.

Participate in the education of pre and post registered nurses, medical staff, AHPs, patient and carers and participate in curriculum development where appropriate.

Clinical Governance

Promote patient and public involvement activities in the specialist area, leading to service improvement.

Human Resources
Observe provisions of and adhere to all Trust Policies and Procedures.
Actively participate in the annual performance review to identify personal development needs.
Attend Trust Statutory and Mandatory training sessions as required and any other training courses relevant to the post.
Specialist Practice
Perform comprehensive assessment of patient nursing needs putting quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions.
Perform advanced clinical skills in assessment and/or diagnosis and/or treatment.
Collect, collate, evaluate and report information, maintaining accurate patient records.
Involve patients and carers/relatives in the planning and delivery of care and development of service.
Monitor investigation results, initiating further assessment/review of treatment pathway as required.
Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to ongoing care needs and discharge arrangements.
Establish and maintain effective communication with patients and carers/relatives and professionals across health and social services.
Contribute to the health promotion agenda with the local health economy.
To undertake non-medical prescribing duties within the legal framework for nurse prescribing and adhere to the Trust's Policy for Non-Medical Prescribing.
In keeping with the NMC 'Code of Professional Conduct' and 'Standards of Proficiency for Nurse and Midwife Prescribers', keep up to date with best prescribing practice in relation to the management of conditions within his/her specialist area
Undertake regular audits of his/her prescribing practice.
Promote self-management, undertaking risk stratification to determine those who can self-manage, following education, those who will need guided support to self-manage and those who will require on-going face to face support.
