Transfer of Care Hub Clinician

Are you an established nurse or AHP looking to develop your skill set and be part of a large transformation project? This is a great time to be part of The Transfer of Care Hub service, sharing your clinical expertise and having your ideas heard, whilst reviewing the discharge process for both the acute and community settings.
As the Transfer of Care Hub Clinician, you will provide a consistent presence within the acute settings, Poole and Bournemouth Hospital on behalf of Dorset HealthCare for the Transfer of Care hub. You will be responsible for clinically triaging referrals sent to single point of access (SPA) within Dorset Healthcare Trust and liaising with all teams involved in planning discharges and agreeing patient pathways. Although you will not be delivering clinical duties you will be contributing and shaping the development of a Transfer of Care Hub within acute hospital sites across Dorset.
This is a full-time fixed term role (until March 2026) covering a seven-day service. We would welcome part time applicants with a minimum of 22.5 hours. We can be flexible with how these hours are met, whether condensed hours or spread over the week to support childcare arrangements. Ideally, we would like a 1 in 4 weekend cover at enhanced rates.
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.
- Your role will involve liaising with patients and families on the ward when needed to expedite discharges and support MTD's as part of complex discharge planning
- You will work closely with the Cluster leads, acute hospital teams, social workers and community providers to ensure patients have a smooth transition onto the correct pathway contributing towards discharge and flow across the system.
- To be the consistent presence within the acute hospital for the Transfer of care hub and be responsible for liaising with the acute hospital teams, providers and other services in planning discharges and agreeing patient pathways.
- To triage referrals sent to SPA working with the central SPA team and liaise with Patients and Families on the wards where needed to expedite discharges.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
