Build and maintain strong links with voluntary, community, and neighbourhood groups to map and maintain a directory of local assets, enabling timely, appropriate, and supported referrals. Collaborate with local partners to strengthen and sustain community-based services and groups. Engage in ongoing professional development, actively reviewing and enhancing the scope of the role in line with evolving community needs. Provide tailored advice and support to individuals with memory concerns, ensuring early engagement and access to appropriate services.
Deliver emotional and practical support to patients and families throughout the dementia diagnosis process and as the condition progresses. Act as a consistent point of contact, ensuring continuity of care and fostering trusted relationships with patients and carers. Support carers through signposting, emotional support, advocacy, and by facilitating access to relevant services. Assist veterans and their families in navigating and accessing appropriate healthcare and community-based services.
Promote self-management and independence by providing personalised support that enables individuals, families, and carers to take control of their health, make informed decisions, and improve health and wellbeing outcomes.
