Overview
We are seeking a dedicated and organised Care Coordinator to join our team. The successful candidate will play a vital role in managing client care plans, coordinating services, and ensuring the highest standards of support are maintained. The role involves working closely with clients, healthcare professionals, and service providers to deliver personalised care solutions.
Responsibilities
- Serve as the main point of contact for clients, families, carers, and external professionals to ensure smooth communication and continuity of care.
- Assess client needs and develop personalised care plans in collaboration with clinical staff and support teams.
- Coordinate and schedule services, including home visits, appointments, and multidisciplinary meetings.
- Monitor care delivery, ensuring services are provided on time, meet quality standards, and align with client goals.
- Maintain accurate documentation, including assessments, care plans, progress notes, and regulatory reports.
- Respond promptly to concerns or changes in client condition, escalating issues to appropriate teams when necessary.
- Ensure compliance with safeguarding, confidentiality, and regulatory guidelines (e.g., CQC or relevant governing body).
- Collaborate with healthcare providers, social workers, community organisations, and family members to support holistic care.
- Track outcomes and performance metrics to support quality improvement initiatives.
- Support clients through transitions, such as hospital discharge, new service arrangements, or changes in care needs.
- Provide guidance and information to clients and families about available services, benefits, and support pathways.
Experience
- Prior office experience is desirable but not essential; organisational skills are key
- Strong communication skills, both written and verbal
- Ability to prioritise tasks effectively in a fast-paced environment
- Compassionate approach with a genuine interest in supporting others
- Proficiency in using standard office software programmes (e.g., MS Office)
- Experience working within healthcare or social care settings is advantageous but not mandatory
