Overview

We are seeking a dedicated and organised Care Coordinator to join our team. The successful candidate will play a vital role in managing client care plans, coordinating services, and ensuring the highest standards of support are maintained. Responsibilities

  • Serve as the main point of contact for clients, families, carers, and external professionals to ensure smooth communication and continuity of care.
  • Assess client needs and develop personalised care plans in collaboration with clinical staff and support teams.
  • Coordinate and schedule services, including home visits, appointments, and multidisciplinary meetings.
  • Monitor care delivery, ensuring services are provided on time, meet quality standards, and align with client goals.
  • Maintain accurate documentation, including assessments, care plans, progress notes, and regulatory reports.
  • Respond promptly to concerns or changes in client condition, escalating issues to appropriate teams when necessary.
  • Ensure compliance with safeguarding, confidentiality, and regulatory guidelines (e.g., CQC or relevant governing body).
  • Collaborate with healthcare providers, social workers, community organisations, and family members to support holistic care.
  • Track outcomes and performance metrics to support quality improvement initiatives.
  • Support clients through transitions, such as hospital discharge, new service arrangements, or changes in care needs.
  • Provide guidance and information to clients and families about available services, benefits, and support pathways.

Experience

  • Prior office experience is desirable but not essential; organisational skills are key
  • Strong communication skills, both written and verbal
  • Ability to prioritise tasks effectively in a fast-paced environment
  • Compassionate approach with a genuine interest in supporting others
  • Proficiency in using standard office software programmes (e.g., MS Office)
  • Experience working within healthcare or social care settings is advantageous but not mandatory
