What will your responsibilities be?

In this role, you will manage casework for hospitalized adults, ensuring smooth care coordination and discharge planning. You'll assess needs, develop care plans, and facilitate transitions, while maintaining accurate records and compliance with standards. This position allows you to make a significant impact on patients' recovery and well-being.

Essential Requirements:

  • Social Work England registration
