Job Overview
The Agnes Unit is a 12 bedded assessment & treatment inpatient unit for people with learning disabilities aged 18 and over who have mental health problems, pervasive developmental disorders and challenging behaviour.
The Band 3 HCSW role involves having good relationships with patients and up to date knowledge of care plans, as well as providing leadership in the clinical areas to other members of the team.

Our services to patients are on a 24/7 basis and you will be expected to work a variety of shift patterns. We try and accommodate individual needs through flexible working, but cant guarantee we can accommodate every request.

Job Description Summary (Main Duties And Responsibilities)
- To support the named nurse with direct and indirect tasks related to patient care.
- Support patients on escorted leaves
- You will be involved in long-term planning of care. This involves taking observations from direct clinical work, as well as the experience of staff which will be included in long-term care planning meetings with the named nurse, to help develop care plans, activity plans, strategies, and approaches to working.
- Support the patients activity programme, working alongside the therapies team and ensuring these plans are implemented outside of therapy staff working hours
- Ensuring care plans are followed
- Complete additional courses required for the role including clinical and leadership courses
