Job Title: Step Out Support Worker
Responsible to: Resource Manager
Contract: 35 Hours per week over 4 days. Saturday working as part of a rota system. Additional hours may be available.
Salary: £24,243 per annum pro rata for 35 hours per week
Pension: Entry into the Age UK Stockport Auto Enrolment Scheme
Location: Whitehill Resource Centres, Reddish
MAIN PURPOSE OF THE POST:
To support the delivery of high quality supportive and caring day service activities in a variety of settings.
To support the co-ordinator to implement personal support plans including risk assessment, moving and handling assessments and any other social care requirements.
To support the coordinator to create a supportive and enabling environment where the customers can achieve maximum independence, and / or regain previous levels of functioning, to focus on ensuring positive outcomes for customers which allows them to participate and contribute to their local communities.
To undertake all essential duties to ensure the customer remains safe and well at the chosen activity/ venue.
To assist in the ongoing assessment and rehabilitation of customers and report any issues or concerns to the appropriate person
To support with kitchen duties to include heating & serving meals, complying to food safety regulations.
Main Tasks and Responsibilities:
The following details reflect the content of the post at the date prepared. The post holder will be expected to adopt a flexible attitude to the duties which may have to be varied, after discussion with the post holder, subject to the needs of the newly restructured Step Out Stockport team. Consequently this job description may be revised from time to time.
A DELIVERY – To support Step Out Stockport:
To support the co-ordinator in:
- Ensuring safe and appropriate support including personal care and moving and handling to customers in their activity.
- Identifying the needs and wishes of groups and individuals and developing and tailoring activities in response to enable people to achieve their individual outcomes
- Organising and co-ordinating activities for active daily living
- To work in partnership with customers, carers colleagues and staff from other agencies.
- Providing as much or as little support to each individual as they require enabling them to develop ongoing activities.
- Ensuring that equality and diversity issues are positively recognized by working with diverse groups to ensure equal opportunity to all people from all backgrounds
- Working co-operatively and supportively to find ways of overcoming barriers to and facilitating people involvement and continued membership
- Providing emotional support and assistance and hands on practical support to customers as required
- Deliver standards of care including dignity, equality and diversity.
B ADMINISTRATION
To support the co-ordinator to:
Maintain appropriate administrative systems to ensure effective delivery and individual outcomes
Gather and collate statistical and other information and data as required, ensuring the effective qualitative and quantitative monitoring and evaluation of the service
Handle and process any money in accordance with organisational financial policies and procedures.
Follow appropriate policies, procedures and practice guidelines for the project to ensure good practice
C ACCOUNTABILITY
Keep updated on developments and good practice relevant to the work and attend training events and courses as required.
To attend individual supervision meetings and team meetings as required.
Ensure that all work is undertaken in accordance with the aims, values and policies of Step Out Stockport, including Equal Opportunities, Confidentiality and Health & Safety.
D OTHER
To work alongside other staff and volunteers in a positive way.
To positively promote and represent Step Out Stockport at all times.
To carry out such other duties as deemed appropriate and may be reasonably required by the Company Secretary
