What You'll Do as a Healthcare Assistant/Health Coach

The Health Care Assistant (HCA)/Healthcoach plays a critical role in supporting Virtual Care by assisting in the monitoring and care of patients remotely and also to help people develop the knowledge, skills and confidence to manage their own health and wellbeing, supporting them to stay well.

The post holder will use the web-based Doccla platform to help monitor patient wellbeing, support their care needs, and assist in delivering high-quality care in collaboration with the wider clinical team.

The purpose of the health coaching element of this role is to help people develop the knowledge, skills, and confidence to manage their own health and wellbeing and support them to stay well. You will deliver effective health coaching sessions using a holistic and person-centred approach that helps people focus on what matters to them. The aim is to empower people to take control of their own wellbeing by building their confidence, motivating them to set goals and creating personalised action plans.

HCAs/Health coaches will use health coaching, motivational interviewing and behaviour change skills to empower people to make positive lifestyle changes and provide support to ensure they complete evidence based interventions such as vaccinations, regular exercise, stopping smoking, achieving/maintaining a healthy weight and completion of pulmonary rehabilitation.

The role will take a holistic approach that helps increase people's ability to access community support and self-management education and connect to Social prescribing and other support services available. Health Coaches will work alongside clinical colleagues in the health hub to provide a joined-up approach, maximise patient experience and reduce escalations in care

  • Assist in the monitoring of vital signs and collection of patient data through remote technology.
  • Provide support to patients in their home environment, ensuring their comfort, safety, and wellbeing.
  • Communicate effectively with patients, offering reassurance and advice as directed by clinical protocols.
  • Support the clinical team in managing patient care pathways, ensuring timely responses to any patient needs or concerns.
  • Assist in maintaining accurate and up-to-date patient records.
  • Escalate patient concerns to the appropriate healthcare professionals as per established protocols.
  • Participate in clinical audits and quality improvement initiatives.
  • Provide general administrative support to the clinical team, including managing appointments and patient follow-ups.
  • Support the integration of care pathways for remote patient monitoring to enhance patient outcomes.

  • Registered Manager CQC
  • Key account managers
  • The commercial team

What Experience You'll Bring to the Team

The post holder will be a compassionate, proactive individual with excellent communication skills and a keen interest in patient care. They will be comfortable working in a flexible scale-up environment.

Qualifications:

  • NVQ Level 3 in Health and Social Care or equivalent experience (desirable)
  • Current Care Certificate (desirable)
  • Right to work in the UK

Essential:

  • A minimum of 6 months of experience as a Health Care Assistant in a clinical setting.
  • Experience in patient monitoring and basic care provision

Desirable:

  • Experience in remote patient monitoring or virtual wards.
  • Familiarity with digital healthcare platforms.
  • Experience working in a start-up or rapidly changing environment.

Location:

  • The Healthcare Assistant/Health Coach role is home-based. However, the post holder is expected to travel to the London office at least one day per month for clinical supervision and team meetings. You will also be required to attend client visits to various NHS hospitals and community hubs.
