Summary
The Advanced Clinical Practitioner (ACP) will provide high-quality, patient-centred assessment, diagnosis, treatment, and management of individuals presenting with a wide range of injuries and illnesses within urgent and unscheduled care settings.
Working autonomously, the ACP will deliver safe and effective care within agreed local and national guidelines, drawing on advanced clinical reasoning and evidence-based practice. The role includes independent prescribing, complex decision-making, and holistic patient management across all age groups, from paediatrics to older adults.
The ACP will work collaboratively within a multidisciplinary urgent care team, contributing to service development, quality improvement, and clinical education.
Working Pattern and Rota Expectations
- This is a full-time clinical role, with a minimum commitment of 24 hours per week.
- The urgent care service operates 24 hours a day, 7 days a week, including weekends and bank holidays.
- Clinicians are expected to work a variety of shifts, including days, evenings, nights, and weekends.
- A minimum of one weekend per month is required as part of the rota.
- Shifts are not fixed; flexibility is essential to support safe and effective service coverage.
- Rota schedules are released 12 weeks (3 months) in advance to support work–life balance and planning.
- Requests for flexible working patterns will be considered where possible, in line with service needs.
Responsibilities
Clinical Practice
- Independently assess, diagnose, treat, and discharge or refer patients presenting with minor illness and injury.
- Manage undifferentiated and complex presentations using advanced clinical reasoning.
- Prescribe, supply, and administer medicines within the scope of independent prescribing and local protocols.
- Undertake both face-to-face and remote consultations, ensuring safe management across all age ranges.
- Initiate and interpret investigations such as, ECGs, and imaging as appropriate.
- Work autonomously within scope of competence while maintaining collaborative relationships with medical, nursing, and AHP colleagues.
- Provide urgent and emergency interventions when required, in line with resuscitation and escalation procedures.
- Maintain accurate, timely, and comprehensive clinical documentation.
Professional Standards
- Maintain registration with NMC, HCPC, and uphold the relevant Code of Professional Conduct.
- Take accountability for clinical decisions, ensuring care is evidence-based and patient-centred.
- Adhere to all relevant FedBucks and regulatory policies and standards, including safeguarding, confidentiality, and information governance.
- Engage in reflective practice, professional supervision, and continuous development to maintain advanced competence.
Governance, Quality, and Safety
- Participate in clinical audit, service evaluation, and incident review to support ongoing improvement.
- Contribute to the development and review of clinical pathways, guidelines, and protocols.
- Promote a culture of patient safety, quality, and compassionate care.
- Support effective communication and collaboration within the team and with external partners.
Education and Development
- Maintain and develop advanced clinical knowledge through structured CPD and reflective learning.
- Participate in the teaching, mentoring, and supervision of junior staff and learners where appropriate.
- Support the development of clinical competencies across the urgent care team.
- Engage in service evaluation or research projects that improve patient outcomes or practice efficiency.
- Complete all mandatory and role-specific training as required.
Essential Qualifications
Registered Clinician with NMC/HCPC
Master's degree in advanced clinical or Nursing Practice (Equivalent or higher)
Minor Injuries and illness qualification
Independent prescriber

EssentialExperience
5 years post registration experience of which 2 years will have been spent in primary care, ED, unscheduled care, or related field.
Experience managing both adult and paediatric presentations.
AdditionalExperience
Previous experience in GP OOHs, walk-in centre, or MIU
Experience in clinical guideline or pathway development
Special skills and knowledge
Advanced assessment, diagnostic and decision-making skills
Excellent communication, documentation, and digital literacy
Competence in both illness and injury management
Knowledge of clinical governance, safeguarding, and infection control
