Dementia Ambassador

Manton Heights Care Centre are requiting for a Dementia Ambassador to join their team to enhance the lives of the people living at Manton Heights.
Key Objectives:
- Take a lead for quality and improvement in dementia care delivered across the RCH care services, in accordance with the agreed dementia strategy.
- Promote positive attitudes to working with people with dementia and their family and friends who support them, and key stakeholders involved in their care
- Act as an Ambassador for the care centre communicating with relatives, prospective customers and the wider community in a way which is consistent with our values.
- Build the capacity and resilience of staff to respond effectively to the strengths and needs of residents with dementia by use of formal and informal supervision, mentoring and role modelling.
- Build effective partnerships with care professionals, including GP's, CMHT, DN's and local authority placement teams
- Work closely with the Regional Dementia Manager in the delivery, audit and review of the group dementia strategy.
Responsibilities
- Lead on the delivery of projects and initiatives that will enable the care centre to meet the group's dementia standards.
- Work with the home manager to audit and evidence the progress of the care centre in meeting the group's dementia standards.
- Introduce specific assessment tools such as the Pool Activity Level approach and needs based behaviour models. Use RELISH, our digital platform, to focus on those highlighted to be at potential risk of isolation.
- Ensure a high standard of personalised care delivery, based on the assessment of a person's strengths and care needs using RCH profiling documents, and in consultation with residents, their relatives, and the care team.
- Liaise with GP's, prescribers and other healthcare professionals linked to the service by sharing the tool 'Optimising Treatment and Care for Behavioural and Psychological Symptoms of Dementia' and discuss medication reviews.

Person specification
- NVQ Level 4 required/or ability to work to
- Extensive Dementia experience and extended Dementia qualifications are essential.
- At least two years' experience in long term care for older people, preferably experience caring for individuals with dementia in a care home / nursing home.
- At least one year of supervisory and management experience
- Excellent communication (written and verbal)
- Passionate and keen to develop new skills.
- Ability to organise efficiently and prioritise workloads whilst under pressure
