Overview
We are seeking for a dedicated and skilled Care Coordinator to join our healthcare team. The successful candidate will play a vital role in ensuring seamless communication and coordination of care services for our clients. This position offers an opportunity to make a meaningful difference in patients' lives while working within a supportive and professional environment. Prior experience is preferred, as it will aid in managing tasks efficiently and effectively.
Duties
- Act as the primary point of contact for clients, healthcare providers, and care teams to facilitate smooth communication and service delivery.
- Schedule appointments, follow-ups, and coordinate care plans in accordance with client needs and healthcare protocols.
- Maintain accurate and up-to-date client records.
- Assist with administrative tasks such as data entry, filing, and managing correspondence related to care services.
- Collaborate with multidisciplinary teams to ensure all aspects of client care are coordinated effectively.
- Provide support to clients by answering queries, offering guidance, and ensuring their needs are addressed promptly.
- Monitor service delivery progress and escalate issues when necessary to ensure high standards of care are maintained.
Qualifications
- The ideal candidate should hold a Level 5 Diploma or NVQ Level 3 qualification (or equivalent) in Health and Social Care or another health-related field, with several years of hands-on experience.
- Proven experience in an office environment or administrative role is highly desirable.
- Excellent organisational skills with the ability to manage multiple priorities efficiently.
- Strong communication skills, both written and verbal, with a compassionate approach towards clients.
- Proficiency in office software applications such as Microsoft Office or similar programmes.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Attention to detail and accuracy in record keeping and documentation.
- A genuine interest in healthcare or social services is advantageous but not essential.
