Role Overview:

The role of the Case Manager is central to a clients' journey throughout their treatment at Orri. The role ensures clients have a consistent experience by coordinating all aspects of the clients care across the MDT and provides a single point of contact for the client, the people who support the client and for professionals both within the service and externally. They work with the client to support them from admission into induction and stay alongside them throughout their time in Orri up to discharge.

Main Duties:

  • To introduce themselves to the new client and their carer/friend/partner (where appropriate) on admission into Orri.
  • To ensure new clients receive their full induction by the clinical team.
  • To ensure all information such as address, contact numbers/email, emergency contact, consents, carer, allergy information and GP information is correct and accurate.
  • To organise 6 weekly Client Review Meetings (in week 4 of each treatment block) with clients to discuss the previous block of treatment and the planned next block of treatment.
  • To listen to and understand client's concerns that come up within reviews and discuss these, where appropriate, with the client's care team feeding back to the client outcomes of those discussions. The Case Manager will ensure all correspondence / summary of discussions is stored in the clinical electronic medical records system.
  • To attend and participate in the Multi-Disciplinary Team review to receive feedback from the clinical team on the appropriate next course of treatment for the client. The Case Manager will then use this information to inform the Client Review Meeting.
  • To ensure all clients in their caseload have an active treatment plan, have a Client Review Meeting planned for each treatment block and have a new Treatment Plan signed off by week 5 of their previous blocks.
  • When a client is approaching discharge: plan a discharge meeting, ensure discharge paperwork is commenced and completed, and ensure a Discharge Meeting has taken place. This can include members of the Orri care team/ family members / carers / external teams (as appropriate), all of which is coordinated by the Case Manager.
  • To ensure Discharge Paperwork is finalised, signed and sent on the day of discharge.
  • To attend the weekly Register Meeting to report on any issues with their caseload.
  • To ensure the client and the carer (if relevant) are fully informed at each Review Meeting of the financial implications of treatment and key aspects of the Terms and Conditions.
  • The Case Manager will be the first point of contact for all queries related to requests to change Treatment Plans and ensure that the Terms and Conditions are considerately and accurately explained to the Client.
  • If the client is using Private Medical Insurance, ensure reviews are held in good time and applications are made to avoid any treatment pauses due to administrative delays.
  • To ensure the Client understands the importance of filling out outcomes forms (DASS, EDEQ) and to encourage participation.
  • To summarise trends in the client's outcomes to the client in Review Meetings.
  • To manage and resolve concerns with the Client as and when they arise, and to escalate appropriately should they become a complaint.
  • To hold the boundary with the client regarding what is required for the next block of treatment and discuss any requested changes with the clinical team to ensure these are clinically appropriate. If they are not the Case Manager will need to communicate the reasons to the client.
  • To carry out a monthly audit on their Case load to ensure all the correct documentation has been completed and signed.
  • To collate trends and themes each quarter on their caseload to feed into service development.

  • Represent the care team's view in Client Review Meetings.
  • Maintain a professional manner at all times and provide high level of customer service to clients.
  • To ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity in accordance with the provisions of the Data Protection Act and its amendments.
  • To ensure skills are up-to-date and relevant to the role, to follow relevant Orri policies and professional codes and to maintain registration where this is a requirement of the role.
