CARE COORDINATOR – JOB DESCRIPTION

Location: Hemel Hempstead (Office-based with community visits as required)

Salary: Starting: £28,000 per year

Contract: Full-time, Permanent

Reports to: Registered Manager, Director

Driving Licence: Mandatory – must have own car (Paid Mileage for work related travel)

Experience: Minimum 1–2 years in care coordination (or similar role/ senior position within care setting)

Qualifications: Level 3 Diploma in Health & Social Care (or working towards)

About the Role

We are looking for a motivated, organised and compassionate Care Coordinator to join our growing team at Huldra Recruitment & Care. This is a crucial role in supporting both our clients and our care team by ensuring the smooth delivery of outstanding, person-centred care across all services.

You will be responsible for rota management, care planning, communication with social services and families, and supporting the operational excellence of the service. This is an office-based role, with regular visits out in the community for assessments, reviews and quality checks.

Key Responsibilities

Rota & Scheduling Management

  • Create, update and manage weekly rotas for all carers ensuring continuity and fairness.
  • Ensure schedules meet the needs of clients, staff availability, and compliance requirements.
  • Respond promptly to any changes in service: increases, decreases, cancellations, time adjustments.
  • Reallocate staff during sickness, emergencies, lateness or unplanned absences.
  • Ensure all visits are covered in line with contractual requirements and internal standards.

Client & Service Coordination

  • Book in new clients, schedule initial visits and assessments.
  • Communicate clearly with social services, clients, families and carers regarding care arrangements.
  • Forward all relevant client updates to the Registered Manager and team members.
  • Conduct initial assessments, risk assessments, and compatibility checks.
  • Complete and update person-centred care plans and risk assessments.
  • Support with quality assurance visits and client reviews.

Operational Duties

  • Implement and manage new systems and processes as directed by management.
  • Maintain accurate digital and paper records in line with regulatory standards.
  • Support with compliance documentation, audits, and training logs.
  • Liaise closely with the Registered Manager on safeguarding, incidents and care concerns.
  • Escalate clinical or serious issues in line with policies and procedures.
  • Maintain excellent relationships with external stakeholders, including social workers and commissioning teams.

Staff Support & Leadership

  • Provide carers with clear instructions, updates and support.
  • Carry out spot checks, competency assessments and supervisions when required.
  • Assist with recruiting carers, conducting interviews, and supporting onboarding.
  • Monitor punctuality, performance and adherence to company policies.
  • Support the team with guidance, conflict resolution and wellbeing when needed.

Emergency Cover

  • In exceptional circumstances, provide frontline care to ensure continuity of service.
  • Assist in resolving last-minute rota gaps or service disruptions.

What We're Looking For

Essential

  • Minimum 1–2 years experience as a care coordinator or similar role in health & social care.
  • Level 3 Health & Social Care Diploma.
  • Strong rota-building and organisational skills.
  • Confident communicator with excellent written and verbal English.
  • Experience dealing with councils, commissioning teams or brokerage.
  • Ability to work under pressure and manage competing priorities.
  • Full UK driving licence and access to your own car.
  • Strong IT skills (Microsoft Office, care planning/rota systems).

Desirable

  • Experience with Log My Care, Birdie, CarePlanner or similar systems.
  • Knowledge of CQC regulations and compliance.
  • Background in domiciliary care, supported living or complex care.
