Job Purpose

The Senior Care Coordinator supports the Registered Manager in ensuring the smooth running of the branch, particularly in the delivery of complex care, rota management, compliance, and digital monitoring.

This is a hands-on role where the postholder will carry out the full responsibilities of a Care Coordinator, but with additional senior duties. It is designed for someone with the ability and ambition to progress, act as a deputy when required, and provide strong operational support across the service.

The ideal candidate will be highly organised, experienced in complex care coordination, confident with problem-solving, and capable of stepping into leadership tasks when needed—while still being very active in day-to-day coordination work.

Key Responsibilities1. Senior Coordination Duties (Hands-On Role)

  • Carry out all standard Care Coordinator duties (assessments, care planning, rota building, reviews, communication).
  • Take ownership of more complex packages to support safer delivery.
  • Handle escalations and more complex client concerns before they reach the Registered Manager.
  • Provide guidance to other coordinators and carers when required.

  • Deputy Support (When Required)

  • Step in for the Registered Manager during short-term absences for operational continuity.

  • Support with decision-making and prioritising urgent issues.
  • Assist with allocation of work across the coordination function when the service is under pressure.
  • Represent the Registered Manager in meetings when delegated.

  • Client Care Management

  • Conduct initial assessments, risk assessments and care plan updates.

  • Ensure person-centred, clinically appropriate plans are maintained.
  • Attend MDTs, client reviews and professional meetings.
  • Build strong relationships with families and professionals.

  • Rota Planning & Continuity

  • Build safe and efficient rotas, ensuring the correct skill match for complex tasks.

  • Handle urgent rota changes, sickness and continuity risks confidently.
  • Support the team with escalation management during busy periods.

  • Clinical Oversight (Non-Clinical Leadership Capability)

  • Ensure carers are appropriately trained and competent for PEG feeding, tracheostomy care, suction, stoma care, catheter care, ventilation (NIV), NG/NJ feeding, and end-of-life care.

  • Review clinical documentation (MAR charts, notes, observations).
  • Escalate unsafe practice or risks to the Registered Manager promptly.
  • Liaise with health professionals including District Nurses, CHC nurses, GPs, OTs, SALT and hospital teams.

  • ECM Monitoring (Senior Oversight but Hands-On)

  • Monitor ECM daily and resolve missed/late visits, medication not logged, incomplete clinical tasks, and incorrect or suspicious GPS log-ins.

  • Ensure all actions and follow-ups are logged correctly in Nourish.
  • Conduct weekly ECM audits and highlight patterns needing attention.

  • Safeguarding & Incident Response

  • Support initial fact-finding and chronology building.

  • Escalate all safeguarding concerns to the Registered Manager.
  • Assist with statements and documentation where required.

  • Quality Assurance & Compliance

  • Carry out spot checks and home visits.

  • Audit care plans, risk assessments, daily logs and MAR charts.
  • Support CQC readiness, evidence gathering and quality improvement work.
  • Promote safe practice, professionalism and continuous improvement.

Person Specification

Essential

Desirable

Strong experience as a Care Coordinator in domiciliary or complex care.

Experience supporting managerial duties.

Ambition and capability to progress towards a deputy/management role.

Knowledge of Nourish.

Strong rota management and organisational skills.

Local knowledge of South East London.

Good knowledge of complex care tasks (oversight only).

Experience stepping up in a deputy capacity previously.

Excellent communication with clients, families, carers and professionals.

Confident problem-solver with ability to make decisions under pressure.

Proficient in ECM systems, digital care platforms and Microsoft Office.

Understanding of CQC regulations and safeguarding.

Ability to work hands-on and lead by example.

Working Hours

Full-time, with flexibility. Occasional out-of-hours support may be required during service pressures.
