A great opportunity is available to join our thriving, friendly, high achieving practice at Berrycroft Community Health Centre. We're looking for an enthusiastic team player to join our Data Quality team, for 25 hours per week.
Main duties of the job
The post-holder is responsible for carrying out all associated administration focused on long term conditions as part of the QOF team to provide data in a timely and accurate manner.
Job responsibilities
- Manage long term conditions administrations and recalls, such as Diabetic, Vascular, COPD and Asthma. This includes sending appointment invitation letters, booking appointments, sending reminder text messages and letters to non-responders and DNAs, as well as managing AccuRx replies and tasks.
- Issue details to appropriate clinicians for monthly care home reviews and patients that are housebound.
- Manage patient annual reviews and make sure associated follow ups are booked with the relevant clinician.
- Arrange appointments for Learning Disability Health Checks. Liaise on a monthly basis how many LD checks are required. This will also involve sending out appointment confirmations and health check questionnaires.
- Monitor PCN Investment and Impact Fund (IFF) Indicators.
- Manage and arrange quarterly appointments for long-term condition patients living in supported accommodation and liaise with relevant clinicians.
- Work closely with Pharmacy Team to maintain Patient Life-cycle.
- Using the CQRS (Calculating Quality Reporting Service).
- Quarterly returns.
- Communicate regarding child immunisations for non-responders in line with QOF requirements, including sending letters, text messages and booking appointments.
- Work closely with the QOF team to ensure data is provided in line with regulatory requirements.
There may be, on occasion, a requirement to carry out other tasks.
Person Specification (Essential)
- Experience of working with the general public.
- NHS/Primary care general practice experience.
- Ability to prioritise and work to tight deadlines in a fast-paced environment

Experience (Desirable)
- GCSE Grades 4-9 or equivalent in Maths and English
- Strong IT Skills (Microsoft Office)
- EMIS user skills
- Flexibility to work outside core office hours
- Full UK driving licence
