Overview:

We are looking for a compassionate, organised Care Co-ordinator to support our supported living, domiciliary, and medication services. You will coordinate care rotas, manage staff, and ensure high-quality, safe, person-centred care in line with CQC standards.

Key Responsibilities:

  • Schedule and allocate care staff according to service user needs
  • Maintain accurate care records, rotas, and electronic systems
  • Support medication coordination and MAR chart compliance
  • Respond to emergencies, staff absences, and short-notice changes
  • Liaise with service users, families, staff, and external professionals
  • Support audits, quality monitoring, and CQC compliance

Requirements:

  • Experience in care coordination, senior care, or supervisory role
  • Knowledge of domiciliary or supported living services
  • Strong organisational, communication, and IT skills
  • Understanding of safeguarding, care planning, and medication management
  • NVQ Level 4 in Health & Social Care (Essential)
  • Full UK driving licence (Essential)
