Care Practitioner – Join the By Your Side Team

Salary: £13.40 – £15.50 per hour (depending on qualifications & experience)

Contract: Full-time & Part-time available

By Your Side is seeking compassionate and dedicated Care Practitioners to join our expanding team. This role involves delivering high-quality, person-centred care to individuals in their own homes or care settings, supporting independence, dignity, and wellbeing.

Key Responsibilities

  • Deliver personalised care in line with individual care plans, ensuring dignity and respect at all times
  • Support with daily living activities including personal care, mobility, and medication assistance
  • Assist with domestic tasks such as meal preparation, cooking, and light housekeeping
  • Accurately maintain care records using IT systems, ensuring confidentiality and regulatory compliance
  • Communicate effectively with clients, families, and colleagues to coordinate care
  • Drive clients to appointments or social outings where required, ensuring safe transportation
  • Monitor clients' health and wellbeing, escalating concerns to healthcare professionals when necessary
  • Adhere to health and safety standards, policies, and procedures

Skills & Experience

  • Compassionate, reliable, and committed to improving quality of life
  • Experience in home care or care home settings is desirable (training provided)
  • Knowledge of personalised care planning
  • Confident IT skills for record keeping and communication
  • Excellent verbal and written communication skills in English
  • Strong organisational skills and ability to prioritise workload
  • Ability to work independently and as part of a team
  • Valid UK driving licence is desirable
