The team is responsible for working with people to assess both the immediate and ongoing needs, in order to support a person in implementing their longer term care and support needs. The team is located within a central community based office but will support discharge planning across the various hospital sites in and out of the borough as part of a multi-agency team to facilitate effective, safe and timely discharges.
