Overview

We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a pivotal role in ensuring the delivery of high-quality care to our residents in a care home setting. This position requires strong leadership skills, effective communication abilities, and a commitment to enhancing the well-being of our senior clients. The Care Coordinator will be responsible for developing and overseeing care plans, supervising staff, and ensuring that all operational standards are met.

Responsibilities

  • Develop and implement individualised care plans tailored to the needs of each resident.
  • Supervise and support care staff to ensure the highest standards of care are maintained.
  • Communicate effectively with residents, families, and healthcare professionals to coordinate care services.
  • Conduct regular assessments of residents' needs and adjust care plans accordingly.
  • Drive initiatives to improve the quality of care provided within the home.
  • Maintain accurate records of resident care and progress, ensuring compliance with regulatory standards.
  • Provide training and leadership to staff members, fostering a supportive team environment.
  • Address any concerns or issues raised by residents or their families in a timely manner.

  • Previous experience in a supervisory role within senior care or a similar environment is essential.
  • Strong understanding of care home operations and regulatory requirements.
  • Proficiency in IT systems relevant to care coordination and documentation.

  • Demonstrated leadership capabilities with a focus on team development and support.
