We are looking to recruit a warm and friendly Patient Navigator to join our team.
Fireclay Health is a vibrant, dynamic and happy place to work. We provide health care from our two established practices at St George Health Centre and Lodgeside Surgery, the surgeries are close to each other and we offer a full complement of general practice services.
You will be joining a supportive and hardworking team in a fun and friendly working environment. There is so much to learn with a new job, so we ensure a structured induction plan is provided with ongoing training and support.
The role is fast paced, it involves significant patient contact over the phone and face to face so excellent communication skills, a professional approach and computer literacy is essential.
Key Responsibilities & Tasks:
- Assist and direct patients to ensure they access the appropriate service or healthcare professional
- Book appointments face-to-face and over the telephone in our call hub
- Contact patients to pro-actively book appointments and check attending an appointment
- Take messages, ensuring accuracy of detail
- Recording patient information via the IT systems
- Provide patients with information regarding test results and other investigations
- Deal with all general enquiries, explain procedures
- Ensure that all new patients are registered promptly and accurately
- Provide general assistance to the Practice team and other visitors via the telephone
- Administration tasks
Undertake any other additional duties appropriate to the post as requested by The Management Team.
Shift patterns are:-
Monday to include 1/2 hr unpaid break) 10.5hrs
Tuesday to include 1/2 hr unpaid break) 10.5hrs
Friday to include 1/2 hr unpaid break) 10.5hrs
Job Types: Full-time, Part-time
Pay: £12.21 per hour
