Job overviewAn exciting opportunity is available for an experienced Community Clinical Support Worker to join our MSNAP Accredited Memory Assessment Service working 30 hours per week.
The Memory Assessment Service East (CMAS) is nurse-led with a strong emphasis on multi-disciplinary team working to ensure that users of the service receive high quality, compassionate care throughout their memory assessment process. You will work within a full MDT which includes nurses, OTs, support workers, a Consultant Psychiatrist and Clinical Psychology, and which is well supported by admin.
The service is proud of being dynamic, responsive and proactive to the changing needs of the local population and its service users.
The team is currently embracing the opportunity to be part of a rebrand and Older Persons transformation of these services over the next few months making this a fantastic time to be part of the development of this ambitious new plan.
Main duties of the jobThe successful candidate will join a service with a positive team culture, focused on learning, developing, improving and supporting each other as well as our service users.
Support workers provide practical and emotional support to patients and their families at different stages throughout their journey through the memory service. They help with day-to-day care, well-being, and activities to promote independence and quality of life.
Clinical physical observation checks particularly if medication has started (e.g. BP, Pulse, general observations) this is then feedback to the relevant clinicians as needed. Support with assessment process and complete functional assessments where clinically indicated. In addition, will support with joint visits with clinicians where clinically indicated.
Undertake various administration tasks such as clinical notes, phone calls and letters using our main EPR System. Working closely and liaising with other Healthcare Professionals and Third Sector services for the benefit of the patient. Will support with attending their CT head Scan appointments if they are unable to attend themselves.
Complete post diagnostic support visits. This covers a range of areas such as emotional and practical support and to signpost accordingly. Supporting the facilitation of post-diagnostic groups (Carers Forum, Carers workshop, Living Well with Dementia Group and MCI)
You must be able to travel to various locations across Suffolk.
