Job description:
The Block Bed Coordinator will oversee the effective management of the Bromley Council block bed contract. This role is responsible for ensuring smooth admissions and discharges, high-quality care planning, and maintaining strong relationships with stakeholders including families, healthcare professionals, and commissioners.
The postholder will act as the key point of contact for all block bed residents and ensure the service operates to the highest standards of care, compliance, and efficiency.
Key Responsibilities
Resident Management
* Complete pre-admission assessments and ensure the environment is fully prepared for new admissions.
* Develop and maintain individualised care plans and risk assessments for all block bed residents.
* Coordinate and manage the admission and discharge process, ensuring seamless transitions.
* Carry out regular care reviews with residents, families, and professionals.
* Oversee the day-to-day operational management of block bed residents, maintaining accurate records of who is in the service at all times.
Stakeholder Liaison
* Act as the main point of contact with Bromley Council, discharge teams, GPs, social workers, and families.
* Attend and contribute to multidisciplinary meetings, ensuring timely communication and follow-up.
* Provide regular updates and reports to internal management and external stakeholders as required.
Quality & Compliance
* Conduct quality assurance surveys with residents and families, feeding back learning into practice.
* Ensure all documentation, records, and reports are maintained in line with company policies and CQC standards.
*Proactively manage and resolve complaints and concerns, escalating where appropriate.
* Support compliance with safeguarding, health & safety, and infection control policies.
Operational Oversight
* Monitor and report on block bed occupancy levels and performance against contract requirements.
* Contribute to continuous service improvement, suggesting and implementing enhancements to care delivery and operational processes.
* Provide line support and guidance to staff directly involved with block bed residents.
Person Specification
Essential
* Previous experience in care home, nursing, or domiciliary care setting.
* Strong knowledge of care planning, risk assessment, and compliance requirements.
* Excellent communication and interpersonal skills, able to build strong relationships with families, professionals, and local authority teams.
* Organised, proactive, and able to manage multiple priorities in a busy environment.
* Strong problem-solving skills and ability to deal with complaints effectively.
Desirable
* Experience of managing block contract or discharge-to-assess beds.
* Knowledge of CQC regulations and quality frameworks.
* Previous leadership or coordination experience within health & social care.
